Instructions

1. Fill out the form honestly and completely. Don't attempt to lie in ANY fashion.
   
   This includes:
   • Intentional "typos" on your name, date of birth, or other data.
   • Use of anything other than your FULL LEGAL NAME. 
     • The use of nicknames or initials are NOT acceptable.
   • Not revealing the true nature of your employment.
     • If you are self employed, tell us the nature of your business.
     • If you work for a local, state, or national government, let us know what department.


2. We DO NOT blindly accept provider references or ANY other screening services. This information solely used in our verification practices.


3. Applicants under the age of 30 will not be accepted.


4. Any phone numbers that you have listed on the screening form WILL NOT BE CALLED. They are for verification purposes only. Clients are responsible for the initial contact with Dreams Escorts in order to book an appointment.
